Daniel Czermak, esq.
Founder Operating Principal
Daniel Czermak is the founder and operating principal of BoldAge.
He founded BoldAge to deliver scalable, integrated solutions that realize the full power of the Programs for All- Inclusive Care (PACE) model.
Daniel brings decades of trailblazing healthcare leadership to BoldAge. He has been intimately involved in starting several successful businesses in the healthcare and high-technology sectors, and currently acts as president of AcuteCare Health System in Lakewood, New Jersey.
In this capacity, Daniel is responsible for all aspects of company development and management and has overseen its growth into a $20 million, employee business.
In , Daniel led AcuteCare Health System’s expansion into PACE through the launch of the Beacon of Life PACE program in Monmouth County, New Jersey. Nationally recognized for patient satisfaction, Beacon of Life has grown to over participants, and in December , it is expanding into Ocean County, New Jersey.
Prior to his current roles, Daniel’s business experience includes a tenure as CEO of Rein & Co., a Lakewood-based management consulting firm serving the high-tech industry.
Seasons hospice in pasadena A Chicago-based journalist who has covered health care and public policy since , his personal interests include fire performance, the culinary arts, literature and general geekery. Seasons recognized that while it would need to play an upward role, and AccentCare thought the same way. The fiscal policy has engaged quality and value in a way that the whole health care continuum thinks of hospice as a complement to quality and cost. Twenty-two years later, we find ourselves with 29 locations in 19 states from coast to coast.In this role, Daniel was responsible for bringing together key executive leadership from both Fortune companies and venture-backed start-ups. He also served as a venture partner in Rein Capital, a $50 million venture capital practice in Lakewood focused on early-stage investment in the technology sector. Before joining Rein Capital, Daniel was an associate at Yankowitz, Goldsmith & Sayers, focusing on corporate and real estate law.
He holds a law degree from Rutgers University, Newark, New Jersey, and a bachelor’s degree in finance from Touro University, New York.

Todd Stern’s multi-decade career in providing post-acute and institutional healthcare solutions began long before his first job in the healthcare industry; his family has been passionately committed to providing post-acute care for three generations.
In addition to being founder & principal of BoldAge, Todd provides stewardship and operational leadership to several healthcare organizations across the post-acute community-based healthcare services ecosystem, including serving as the CEO of Seasons Hospice & Palliative Care.
Under Todds leadership, Seasons Hospice & Palliative Care has grown to become one of the leading hospice and end-of-life care providers in the nation, operating coast to coast and across 20 states.
This depth and breadth of connected healthcare leadership and expertise is unmatched.
A former member of both the National Hospice and Palliative Care Organizations (NHPCO) Public Policy Committee and the Hospice Action Network (HAN) Board, Todd is highly regarded and active within hospice advocacy.
Beyond hospice, Todd co-founded and serves on the board of various healthcare operating businesses focused exclusively on post-acute care with an emphasis on care in the home, including home healthcare, homecare, home-based dialysis, ambulance, home medical equipment, outpatient physical therapy clinics, long-term care and assisted living.
Todd holds a BBA and MBA from Loyola University Chicago and is a certified hospice administrator.
Mary Austin, MSN, NHA, RN

Mary Austin is the CEO of BoldAge.
She has over three decades of professional experience in senior services, including over 15 years in Programs for All- Inclusive Care (PACE).
Todd stern seasons hospice Technology is rapidly growing. Todd: We are watching many different changes occur, most of which are driving a consistent message of increased quality from the healthcare provider while doing it all at a decreased cost. There are a lot of industries that are competing for talent. This required a lot of creativity, leveraging of technology, and prioritizing infection risk with care and also bereavement support.Mary is recognized for her a collaborative, people-driven approach and a firm understanding of the alignment of quality with financial success in PACE.
Since , Mary served in executive positions in PACE, both at the University of Pennsylvania (Penn) and at NewCourtland Senior Services. At Penn, Mary managed the PACE clinical practices before being appointed (COO) of the program, where she directed census growth to over participants, drove new center development and led overall operations.
Mary also led the NewCourtland LIFE (PACE) organization in Philadelphia from start-up through six years of sustainable growth and successful regulatory audits.
Relentlessly committed to quality, Mary was the chair of the Technical Expert Panel for Centers for Medicare & Medicaid Services (CMS) development of quality measures for PACE.
Mary has served in several leadership roles with the National PACE Association (NPA), including the chair of the Nursing Consortium, and Education and Awards Committees. She has been a PACE technical advisor with the National PACE Association since and has worked with a variety of organizations investigating, developing and operating PACE programs in several states.
Mary received her Bachelor of Science degree from Neumann University, Aston, Pennsylvania, and a master’s in nursing administration from Villanova University, Pennsylvania.
Seasons hospice in orange Marcum: Can you tell us the story of Seasons? When I started in hospice 20 years ago, we were celebrated for our holistic approach and our largely capitated reimbursement model. The essence of what we do is already value-based and high-quality care with the lowest cost at the end of life. To achieve that, we make sure that we have nurses and clinicians who are comfortable and competent in delivering complex care to those who dying.She is on the faculty of Philadelphia College of Osteopathic Medicine and is licensed in Pennsylvania as a registered nurse and as a nursing home administrator. In , Mary became a Johnson & Johnson fellow for the Wharton School of Business of the University of Pennsylvania.
